 Staff will provide services to individuals living in private homes for the purposes of providing supervision and/or assistance that is designed to ensure the individual’s health, safety and welfare or to perform activities of daily living for the individual. These services will be provided in private home and community settings.

 Staff will provide services to help individuals gain, maintain, and improve skills that allow them to live and participate in their local community. These services will be provided in private home and community settings.

 Staff will provide chore services to keep the home clean and in a safe condition. Services include household activities such as meal preparation, laundry, sweeping/mop floors, windows, walls; tacking down loose rugs and tiles; moving furniture in order to provide safe access and egress; ice, snow, and/or leaf removal; and yard maintenance. In the case of rental property, the responsibility of the landlord, pursuant to the lease agreement, will be examined prior to any authorization of service. Services will be provided in the private home of individuals.  

 Staff will provide services to supervise and support individuals living in private homes on a short-term basis for planned or emergency situations, giving the person(s) normally providing care a period of relief that may be scheduled or due to an emergency.
